Ordering Instructions:   Print these instructions first!

• This form gives you easy 4  step ordering. 

  1. Fill in the form

  2. Print it, in your browser window

  3. Submit it, by clicking the "Submit Form"  button

  4. Mail it in, with your US Postal Money Order

• If you click the "Submit" button, at the bottom of the page, it notifies us that you will be ordering.  It DOES NOT place your order.  And if you can't get the form to work, I'd appreciate it if you would email me and let me know, but it doesn't make a bit of difference to your order, so don't worry that your order will be messed up.

• Only by sending a letter with an  enclosed Money Order will you have actually placed your order.  Thanks for your business!
